The Star of Sierra Madre is a military decoration awarded to members of the NCR for exceptional service. Its possible its the NCR's equivalent to the US's Medal of Honor. More likely, as the Sierra Madre is an elusive place of lore and death, the medal may be given in reguards to more specific actions. As the Sierra Madre is considered a legend and myth. Making little other reason for the medal to be named after it questionable.
