Sleepwalking note is a paper note that can be found and obtained within the Commonwealth, in Fallout 4.

This note can be discovered within the Old Gullet sinkhole, and exiting out into a nearby building that has been chained up, and is normally inaccessible from the outside. The Sole Survivor will exit the sinkhole into a basement, where the Sole Survivor will need to travel to the first floor, where the note will be found inside of a chem cooler next to a mattress.
