Losing your ear reduces your Charisma by 1 permanently.

The Masticator can rip your ear off, if he hits you while you have 110 HP or less.

Behind the scenes

The item description is one of the occasions in Fallout 2 where the game implies that you are playing a video game.
Unlike a Mutated toe, you cannot eat your ear, nor force-feed it to anyone else.
This is a reference to the infamous boxing incident when Mike Tyson Bit off a portion of Evander Holyfield's ear.
