The title is granted by having sex once if the player character's stats include 9 EN, 10 CH and 8 AG or by having sex with multiple times.
Miria can be exploited to increase the counter if the player character has at least 8 Charisma and convinces Grisham that it's just a medical examination, allowing repeated sexual encounters.
