A fresh apple can restore some Hit Points, without irradiating the consumer. It is one of the few edible food items that doesn't give you any radiation, besides other fresh fruits or vegetables, and food found in a non-compromised vault. Additionally, in hardcore mode, it can reduce Starvation and Dehydration.

Three are available in The Tops presidential suite: 2 near the bar, and one on a shelf in a metal cabinet in the kitchen.
There are five in the Atomic Wrangler, in the kitchen area behind a very hard locked door. They must also be stolen, and if either Garret spots you in the room, they will follow you until you leave.
There is one on a plate at the dining table in the Great Khan longhouse. It must be stolen.
There is one in a box in the mess tent at Camp McCarran. It must be stolen.
Rarely, some traveling merchants may have one in inventory.
