Janice Wilkins is found dead on Vault 101's lower level during the quest Trouble on the Homefront in Fallout 3.

Background[]

A childhood friend of the Lone Wanderer, Janice was a red-headed, quiet child. During the uprising in Vault 101, she was gunned down by the vault's security force, along with her brother, Jim.[1]

Although Janice and her brother can technically be saved before they are gunned down, by rushing ahead of them and distracting the guards, upon returning to the vault, it is discovered that they were killed, regardless of the Lone Wanderer's possible intervention.
