Doctor Barrows is the leader of the Underworld and proprietor of its clinic in Fallout 3.
Doctor Barrows is the informal mayor of the Underworld and proprietor of the Chop Shop and one of the few doctors in the wasteland trying to gain a scientific understanding of the condition of ghouldom. Whenever he is not tending to his patients, he runs experiments on samples gathered from volunteers (skin, hair, occasionally organs) to determine what ghoulification is and whether it can be rectified.[1] He enjoys dark humor greatly, one of the few ways to maintain composure in a taxing line of work.[2][3][4]
Recently, some ghouls scavengers brought a badly mangled Reilly to his shop, after finding her near the entrance to the city. Heavily wounded in a skirmish with supermutants (multiple lacerations, contusions, broken leg, some internal bleeding and blunt force trauma), she has remained in a coma, with Barrows unwilling to wake her up, first to allow her body to heal and second because of the potential for damage to her brain.[5]
Barrows was one of several medical specialists contacted by the Railroad to conduct facial surgery on an escaped android. He dismissed it as a hoax, owing to his experience with appearances and gullibility.[6][7] The fact that he considers medicine to be an important art and wasting it on cosmetic modifications is improper certainly didn't help.[8]
While he tends to crack jokes,[9] he is devoted to his art and will heal anyone who requires attention., even if his patient was a super mutant.[10] Of course, he isn't a charity. He will require payment to cover his expenses.[11][12]
Reilly's Rangers: He is one of the many people who can start the quest. He can wake up Reilly, though if the player doesn't see to it, the Rangers pick her up on the way from the Statesman Hotel.

The Lone Wanderer: "What do you mean "fresh human samples"?" Barrows: "You haven't heard? I'm the foremost authority on Ghoul evolution. I want to know what makes us tick. Something doomed us to this rotting form, and I aim to discover what it is. So, I need samples of human skin, organs and other parts to make the experiments valid." Barrows: "She had multiple lacerations, contusions, broken leg, some internal bleeding and blunt force trauma. She's hasn't been conscious since they brought her in a few days ago. I'd say she tangled with some Super Mutants and lost. It's a miracle she isn't dead." The Lone Wanderer: "How did she end up in the Chop Shop?" Barrows: "She was just brought in by a few others. They found her bleeding to death at the entrance to the city here. If she'd been out in the tunnels any longer, she'd have been someone's meal by now." The Lone Wanderer: "Do you think there's any way to revive her?" Barrows: "Well, I wouldn't recommend it. She's in some sort of coma. It's dangerous to force someone out of one. There's no telling when she'll come out of it I'm afraid." The Lone Wanderer: "Do you mind if I try and revive her?" Barrows: "Unless you're an expert at medicine, I wouldn't recommend it. You may end up doing more harm than good. However, I'm all for helping my patients, so if you think you have the ability, be my guest." (Barrows' dialogue)
